Application Checklist

	Application
	An application must be completed and submitted to Charlottesville Day School as early as possible. (click here to download)
	Application Fee
	A $35 application fee must be submitted with the application. Checks can be made to Charlottesville Day School at ACAC.
	Recommendation Form
	A recommendation form filled out by applicant’s current teacher must be received prior to admittance.
	Transcripts
	Transcripts from prior schools need to be submitted prior to admittance.
	Admissions Visit
	Each applicant will be evaluated to ensure that CDS is an appropriate academic setting for him/her.
	Admissions Meetings
	For each applicant, there will be an initial meeting with the Director of Admissions followed by a meeting with the Head of School.
	Immunization Record
	Current immunization records are required for admittance.
	Birth Certificate
	An original birth certificate must be provided for admittance. A copy will be made and the original will be returned.
	General Agreement & Permissions
	Parents must understand and sign this form prior to admittance.
